Procedure

Online Application
Access the online application via the hyperlink or under the Admissions section and complete the requested information. The form can also be accessed through the Apply Now buttons found in this website.

Entrance Examination and Results
The MMC CAST Entrance Exam is deferred at the moment.

Requirements
Qualified applicants will submit the complete requirements to the Office for Admissions listed below.
- Original PSA Birth Certificate
- Form 138 or Grade 12 Report Card (for incoming freshman)
- Certificate of Grades or Transcript of Records (for transferee & second degree holder)
- Honorable Dismissal (for transferee & second degree holder)
- Certificate of Good Moral Character
- Two (2) copies of 2”x 2” recent colored ID pictures with name tag and white background.
- One (1) long brown envelope

Medical Examination
Chest X-ray results must be submitted to the Office for Admissions..

Enrollment
The Registrar’s Office will review the student’s admission requirements. Once all requirements are verified as complete, the student may proceed with the enrollment process.
